Meredith Crawford, MSSW, CSW, is the primary transplant social worker for the Liver Transplant Program with UofL Health – Trager Transplant Center. Meredith has been with UofL Health for 17 years.
In her role, Meredith conducts complex psychosocial evaluations of potential transplant recipients and living donors to identify barriers to candidacy for transplant or donation. She also provides formal recommendations to the transplant selection committee as to whether a patient is appropriate for a transplant or a living donation. Another core piece of her role is counseling patients and their families related to chronic illness, transplants and caregiving.
Meredith’s two favorite things about UofL Health are that the organization is an academic health care system and that the organization supports team member growth and development.
“We are treating patients with end-stage disease, and it is truly life-changing,” Meredith said. “We provide patients and families with a second chance at life.”
Meredith has big goals for Trager Transplant Center. She hopes that one day the center will be Kentuckiana’s No. 1 choice for transplant care. She lives out the UofL Health mission every day by striving to positively transform health care through compassion, innovation and patient-centered care.
“As a social worker, I provide patient-centered care by meeting patients where they are and advocating for patients to break barriers they have to health care, specifically organ transplantation,” Meredith explained.
Meredith is originally from Pikeville, Ky., but was raised in the southern Indiana and Louisville area. She attended Eastern Kentucky University and University of Louisville for her advanced studies.
One fun fact you may not know about Meredith is that her maiden name is Tudor. Rumor has it that she is a descendant of the royal Tudor family!
