COVID-19 Booster Shots for UofL Health Team Members Who Received the Pfizer Vaccine
Those who work in high risk occupations such as health care may receive a booster dose. With the changing landscape of COVID-19 booster shots, UofL Health is not mandating booster shots at this time. UofL Health team members who received their first two doses of the Pfizer vaccine more than six months qualify for the COVID-19 booster shot.
When it has been at least six months since you received your second dose of the Pfizer vaccine, you will qualify for a booster shot. For example, if you received your second dose of the Pfizer vaccine before April 2021, you will be immediately eligible.
If you qualify for the booster, you can receive the booster from Pharmacy Services at UofL Health – Jackson Street Outpatient Center or at any UofL Health – Urgent Care Plus location. Be sure to bring your original vaccination card. Additional locations will be open for our employees. More information on these locations will be announced soon. Please see the FAQ below for information on appointments.
Frequently asked questions
Where are boosters available?
Pharmacy Services at the Jackson Street Outpatient Center and our urgent care locations are offering Pfizer COVID-19 boosters to any employee who wants it and is eligible for the booster. We do not have the Moderna booster for those that are immunocompromised. Visit https://www.vaccines.gov/ to find a place near you that supplies Moderna booster shots only for those that are immunocompromised.
What should I know before I get my booster?
If you receive your booster shot at Pharmacy Services, we are asking that team members bring in their original card so that we can document appropriately. It is also a good idea to consider your reaction to the second dose when scheduling your booster. Most patients are experiencing a similar, or sometimes lesser, reaction to the booster as the second dose.
What To Do If You’ve Lost Your Vaccine Card
If you’ve lost your vaccination card, there are resources you can use to obtain your proof of vaccination.
To find out if your immunization records are in the Kentucky Immunization Registry, please complete the KYIR Release of Immunization Record and fax it to the registry’s helpdesk at 502-564-4760.
The COVID-19 card you received at the time of vaccination is issued by the CDC. It cannot be replaced with another card.
For more information on immunization records, visit https://chfs.ky.gov/agencies/dph/dehp/idb/Pages/kyir.aspx.
Pfizer Vaccine Earns FDA Approval
As of Aug. 23, 2021, the FDA has provided full approval of the Pfizer COVID-19 vaccine. This is a monumental announcement as we push to get more Kentuckians and our staff vaccinated against COVID-19. As a condition of employment and medical staff privileges, everyone will need to show proof of at least initial vaccination by Sept. 15, 2021. Vaccines can be scheduled at UofLHealth.org/Louisville-Covid-19-vaccinations/. Also, if you have had a vaccine, please be available for questions from our staff who may not have yet gotten vaccination and be a champion for this life-saving intervention in your neighborhoods and communities.
You can find your nearest vaccine location using the state of Kentucky’s Where Can I Get Vaccinated webpage.

Other testing information
- We have reimplemented mandatory COVID-19 testing for all inpatient admissions regardless of vaccination status. Our organization is seeing a higher number of positive tests even among vaccinated patients. While these cases are usually mild, they still present a risk of exposure to patients and staff that have not yet completed their vaccinations.
- We are not currently testing those who are having outpatient procedures or tests unless they present with risk factors or symptoms.
- Repeat testing is not recommended within 90 days of infection except in select circumstances.
- Please continue to monitor patient signs/ symptoms throughout hospitalization. If the patient develops new or worsening symptoms, please notify the provider, as COVID-19 testing may be indicated.
Visitation policies
UofL Health Emergency Department visitor policy update (Aug. 17, 2021)
The last few weeks have been difficult ones regarding COVID-19 for many communities across the United States. We continue to see an increase in the number of positive cases, and we have seen a significant spike in hospitalizations across our region of the US. Within Kentucky this week, we have seen a doubling in the total number of positive COVID cases, and, as a result, more and more patients are seeking treatment at our Emergency Departments and Urgent Care Plus locations. This high number of visits and increasing numbers of patient holds in our emergency departments has led to crowding in these areas making patient care difficult and putting patients and staff at risk as social distancing has become problematic.
Due to these issues, we will be closing UofL Health Emergency Departments and Urgent Cares to visitors beginning today (Aug. 17). Specific policies on how we will communicate with our patients’ families and designated contacts are attached to this email. Additionally, we will still allow a guardian or parent to accompany all pediatric patients and will allow nursing staff to make accommodations for a visitor under extreme circumstances.
We sincerely hope these changes will be temporary, and we are not changing our current visitation policy for our inpatient facilities. We appreciate your patience as we continue to provide care to this fourth wave of the pandemic.
