The Jewish Hospital Hepatitis C Clinic has merged with the UofL Hospital Hep C Center as part of UofL Health.
Effective Monday, May 18, 2020, we will be seeing patients at the Hep C Center.
- Please direct any new referrals to the Hep C Center via fax at 502-681-1371.
- The phone number is 502-561-5687.
- The Hep C Center is located on the third floor of the Jackson Street Outpatient Center (formerly known as the Ambulatory Care Center) at 550 S. Jackson St., Louisville, KY 40202.
The process for referrals is the same: simply fax the referral form and pertinent records. Staff at the Hep C Center will reach out to schedule your patient. After patients are seen, a copy of the office visit note will be sent to you.
Tracy Gowan, APRN, Racheal Huggins, MA and Marilyn Wermuth, coordinator, will be joining Allison Hobbs, APRN, Barbra Cave, PhD, APRN, and Beverly Hart, MA.
