Employee Health is offering flu vaccines at all UofL Health and UofL Physicians facilities and practices.
All employees, contractors, students, vendors, etc., must have documentation of receiving the flu vaccine by Dec. 15. Medical or religious exemption forms and documentation must be submitted by Dec. 15.

If you are a UofL Physicians employee in need of a flu vaccine exemption, please fill out the form that matches your exemption need:
Employees can receive the flu vaccine in their respective Employee Health offices during routine injection hours. Please consult with your Employee Health office if you are unsure of those hours.
If you receive your flu vaccine from outside of UofL Health – Pharmacy Services, please be sure to request a vaccination record from the organization in which you received your flu vaccine. Fax a copy of your flu vaccination record with your employee ID number written on the copy to your facility’s Employee Health fax number:
